How to make an appointment by phone
- Phone 1800 654 324 (ask how we can make this an option to automatically dial the number) to be connected with the Community Care Intake Service to discuss making an appointment.
- All families with a new baby will be contacted directly by a child and family health nurse to arrange your first appointment. If you have not been contacted within 1 week of going home from hospital, please get in touch by phoning 1800 654 324 and telling the Intake Officer you have not received a call from the nurse to arrange the 1-4 week check.
